The Bellingham Central Lions Club in partnership with the Lions Foundation Hearing Program provide an opportunity for individuals in Bellingham to be able to hear and enjoy life to the fullest.
How the Hearing Program Application Works
Follow these three simple steps to apply for hearing assistance through our partnership with the Lions Foundation.

To apply for the hearing assistance program, please download and print both documents below. These forms include the necessary instructions and requirements for your application.
02.
Fill Out
Carefully fill out the application form with your personal and other information. Ensure all sections are completed to avoid delays in processing.
03.
Submit
Once completed, print and mail your documents to our office or scan and email them to our program manager, Erica Buse, for a faster review.
